Abstract

POSSIBILITIES OF DOPPLER ULTRASONOGRAPHY IN VALUING OF MORFOFUNCTIONAL CONDITION OF LIVER FOR PATIENTS WITH VIRAL HEPATITIS AND HEPATOCIRRHOSIS Fedulenkova1,2 Yu.Ya., Vikman1,2 Ya.E. 1Kharkiv national medical university, Kharkiv, Ukraine 2LTD Medical Diagnostic Center «Expert-Kharkov», Kharkiv, Ukraine Abstract. Objective: To study possibilities of dopplerography in the estimation of progress and gravity of chronic viral hepatitis and cirrhosis of liver, and also to define possibilities of dopplerography in the estimation of degree of expression of inflammatory process in a liver from data of ultrasonography. Materials and methods: 50 patients were inspected with the chronic diffuse diseases of liver (34 patients with chronic viral hepatitis and 16 patients with a hepatocirrhosis), and also 15 patients of group of control. The following Doppler indexes were measured: hepatic artery resistive index, hepatic artery pulsativity index, portal vein velocity, modified hepatic index , hepatic vascular index and diameters of hepatic and splenic veins, sizes of spleen, velocity of blood flow in a portal vein, velocity of volume blood flow in a portal vein. These indexes were compared between 3 groups on the degree of inflammation. Results: velocity of blood flow in portal vein, hepatic vascular index and modified hepatic index were the most informative for differential diagnostics of cirrhosis and hepatitis.  Conclusions: Doppler ultrasonography is a highly sensitive method for diagnostics of changes of parameters of blood flow at an inflammatory process and fibrosis of liver.  Keywords: chronic hepatitis, hepatocirrhosis, portal vein, hepatic artery. МОЖЛИВОСТІ ДОПЛЕРІВСЬКОЇ УЛЬТРАСОНОГРАФІЇ ПРИ ВИЗНАЧЕННІ МОРФОФУНКЦІОНАЛЬНОГО СТАНУ ПЕЧІНКИ У ХВОРИХ НА ВІРУСНИЙ ГЕПАТИТ ТА ЦИРОЗ ПЕЧІНКИ Федуленкова1,2 Ю.Я., Вікман1,2 Я.Е. 1Харківський національний медичний університет, м. Харків, Україна РезюмеМета роботи: оцінити можливості допплерографії у визначенні прогресу та важкості перебігу хронічних вірусних гепатитів та циррозов печінки, а також визначити можливості допплерографії в оцінці міри вираженості запального процесу в печінці за даними ультасонографії.  Матеріали і методи: було обстежено 50 пацієнтів хронічними дифузними захворюваннями печінки (ХДЗП) (34 пацієнта з хронічним вірусним гепатитом та 16 пацієнтів з цирозом печінки , а також 15 пацієнтів групи контролю без захворювань шлунково-кишкового тракту. Вимірювалися наступні допплерівські індекси: індекс резистентності і пульсаторный індекс в печінковій артерії, модифікований печінковий індекс, печінковий судинний індекс, а також діаметри печінкової і селезінкової вен, розміри селезінки, швидкість кровоплину у ворітній вені, швидкість об'ємного кровоплину у ворітній вені. Дані індекси порівнювалися між 3 групами згідно запалення і стеатозу. Результати: найбільш показовими були: швидкість кровоплину у ворітній вені, яка дозволяє виділити групу з ХДЗП з групи контролю. Печінковий судинний індекс та модифікований печінковий індекс показові при диференційній діагностиці цирозов та гепатитів.  Висновки: допплерівська ультрасонографія є високочутливим методом діагностики змін параметрів кровоплину при запальному процесі та фіброзі печінки. Двовимірна сонография не дозволяє оцінити прогресування хронічних гепатитів та важкість перебігу цирозов печінки без використання допплерографії. Харьков, Украина РезюмеЦель: оценить возможности допплерографии в определении прогрессирования и тяжести течения хронических вирусных гепатитов и циррозов печени, а также определить возможности допплерографии в оценке степени выраженности воспалительного процесса в печени по данным ультасонографии.  Материалы и методы: было обследовано 50 пациентов с хроническими диффузными заболеваниями печени (ХДЗП) (34 пациента с хроническим вирусным гепатитом и 16 пациентов с циррозом печени, а также 15 пациентов группы контроля без заболеваний желудочно-кишечного тракта. Измерялись следующие допплеровские индексы: индекс резистентности и пульсаторный индекс в печеночной артерии, модифицированный печеночный индекс, печеночный сосудистый индекс, а также диаметры печеночной и селезеночной вен, размеры селезенки, скорость кровотока в воротной вене, скорость объемного кровотока в воротной вене. Данные индексы сравнивались между 3 группами по степени воспаления и стеатоза. Результаты: наиболее показательными были: скорость кровотока в воротной вене,которая позволяет выделить группу с ХДЗП из группы контроля. Печеночный сосудистый индекс и модифицированный печеночный индекс показательны при дифференциальной диагностике циррозов и гепатитов.  Выводы: допплеровская ультрасонография является высокочувствительным методом для диагностики изменений параметров кровотока при воспалительном процессе и фиброзе печени. Двумерная сонография не позволяет оценить прогрессирование хронических гепатитов и тяжесть циррозов печени без использования допплерографии. Karazin Kharkiv National University, Ukraine. Abstract. We investigated periodontium of newborn rats with experimental model of IUGR. Light microscopy showed that microcirculatory response was characterized by a pronounced decrease in vascular density (20,9±18,2 %); presence both contractility and dilatation of the capillary bed. Endotheliocytes of microcirculatory bed are flattened; there are signs of their desquamation. The increasing intravascular blood clotting in the postcapillary and venular portions of the microcirculatory system, along with a partial reduction of the capillary link have been observed.
