Анотація
Актуальність. Національні системи надання Паліативної та Хоспісної Допомоги (ПХД) є важливою частиною цілісних систем охорони здоров’я населення. За рейтинговими оцінками (модифікованою системою оцінки країн Райта М., Лінча Т. та Кларка Д., 2008/2011 рр.; Індекс Якості Смерті, 2015 р.) система ПХД України є такою, що не має ознак системної організації на державному рівні, забезпечує низьке охоплення лікуванням та доглядом паліативних пацієнтів (приблизно 30 % від потреби) та низьку якість лікування (зокрема знеболення) переважної більшості (близька 80 %) охоплених хворих.
Мета. Обґрунтувати модель оптимізованої системи ПХД України на підставі узагальненого досвіду країн з розвиненими системами ПХД та результатів власних наукових досліджень; провести оцінку розробленої моделі.
Матеріали і методи. У дослідженні використані методи системного аналізу та порівняльний. Для оцінки розробленої моделі використаний соціологічний метод: проведене анкетування фахівців з організації охорони здоров’я та ПХД. Результати анкетування оцінені за квартильною шкалою (Q1–Q4).
Результати та висновки. Розроблено модель оптимізованої системи надання ПХД населенню України, яка окреслює суб’єкти та об’єкти управління, мету, стратегію, тактику, функції вдосконаленої системи управління, напрями, методи, заходи, ресурси, систему наукового регулювання. Для вдосконалення існуючого порядку організації ПХД запропоновані наукові, правові, економічні та адміністративні рішення. За параметрами обґрунтованості та послідовності модель визнана експертами високоякісною (оцінка знаходяться в межах Q4[75–100]%). За параметрами прогнозованості, корелятивності та стійкості до змін, а також за загальною оцінкою модель визнана експертами якісною (оцінка знаходяться в межах Q3[50–75]%). Проведена оцінка розробленої моделі дозволяє запропонувати її для використання в організації охорони здоров’я в умовах тривалої реформи системи охорони здоров’я та невизначеності воєнного часу.
